In June 2014, the CQC published a report “From the pond into the sea: Children’s transition to adult health services”, which identified a pressing need for system-wide change, with commissioners and providers of health and social care working together at every level. There are many professionals out there providing excellent transitional care, but as one delegate pointed out at our 2014 Transition event, transition is happening in silos around the country, with many health care professionals unaware of the innovative work going on in other services. This unique event brings you case studies from services that are delivering exemplary models of transitional care.
