This unique NHS paediatric conference will focus on how to improve patient pathways and drive forward care closer to home for children with complex long term conditions. When these children are living longer, this dedicated day will discuss how to improve transition and palliative care processes but also plan for their future to ensure they have the best possible quality of life.
