Confirmed speakers:
Sheila Kasaven qualified as a nurse in 1995 and held many posts up to 2017 within mental health settings including leading the opening of a purpose bult mental health hospital in Lancashire. She took up Clinical Governance posts from 2017 at Manchester Foundation Trust, Bradford Teaching Hospitals and is now the Director of Clinical Governance at Mid Cheshire hospital Trust in Crewe. Sheila is the senior responsible officer for ensuring PSIRF is embedded into the organisation and that Patient Safety is at the heart of the Trust priorities.
Sue Lord qualified as a physiotherapist in 1974 and moved on to Health and Safety and Risk Management in commissioning as well as hospital and community provider settings before retiring in 2012.
Over the last 10 years she has undergone various orthopaedic surgeries as an inpatient and also as a day-case. All these experiences have helped her gain an understanding of the different perspectives of risk in patient care.
As a Patient Safety Partner, she is enthusiastic about continually identifying ways in which patient treatment and care can be carried out safely. This can be by asking questions of clinicians and managers as a “critical friend” of patients, but also recognising that patient and relatives also have a part to play in patient safety.
Tracy worked as a physiotherapist in acute NHS Trusts for 15 years before moving into roles focusing on regulation, quality and clinical governance. She is currently the Clinical Governance and Compliance Manager, and one of the Patient Safety Specialists at Barnsley Hospital NHS Foundation Trust. Tracy is passionate about supporting staff and teams to improve the safety and quality of care and experience patients receive. Embedding the patient safety incident response framework (PSIRF) and a ‘just culture’ are some of her current priorities.
Kristina Murphy is the Patient Safety Specialist at The Dudley Group NHS Foundation Trust; she has over 15 years of experience in Governance within the NHS. Previously, she has worked as a Director within the Special Academy Education sector focusing on strategic development and quality improvement.
In her spare time, Kristina is the CEO and founder of a charity supporting children with access to school uniform; Independent member and Vice-Chair of West Midlands Police and Crime Panel; an Independent Mental Health Lay Manager and a Governor at a Special Academy.
Dan has worked across the NHS, private healthcare and charities since 2010, holding senior positions in Quality and Quality Improvement most recently as the Director Clinical Outcomes and Effectiveness at University Hospitals Sussex NHS Foundation Trust, and as a Trustee with a charity providing inpatient and community care to Adults, young people and Child with life limiting illness. Dan is passionate about continuous quality improvement, safety and patient experience.
